EHR

Electronic Health Record — the digital chart system a clinic uses for clinical care.

An EHR (Electronic Health Record) is the software a clinic uses to document patient care: encounters, vitals, prescriptions, allergies, and clinical notes. Modern EHRs also handle scheduling, billing hand-offs, and patient messaging. "EHR" is often used interchangeably with "EMR" but tends to imply a system designed to share data across providers.
