Provider, health plan, or clearinghouse that HIPAA applies to directly.
A Covered Entity (CE) is one of three types: a healthcare provider that transmits PHI electronically, a health plan, or a healthcare clearinghouse. Most clinics are Covered Entities. CEs are directly bound by all of HIPAA.
