CCD
Continuity of Care Document — a legacy XML summary export of a patient chart.
A CCD (Continuity of Care Document) is an XML-format clinical summary: problems, allergies, medications, immunizations, recent encounters. CCD is a CDA template; many EHRs still emit CCD for transitions of care. Successor: C-CDA.
