Dentsply Sirona

SoftDent (by Dentsply Sirona) is a Windows-installed dental practice management application. Launch it from the desktop icon or Start Menu. Log in with admin or office manager credentials. SoftDent has a classic Windows application interface.

In SoftDent's top menu bar, click "Management". A dropdown will appear. Select "Reports" from the dropdown. The Reports section opens showing report categories.

In the Reports section, click "Patient" or "Patient Reports" in the left sidebar or category list. You will see a list of patient-specific reports.

Click "Patient Listing" — this is the most comprehensive patient data report in SoftDent. It contains all demographic, contact, visit, and insurance information in a single report.

The Patient Listing configuration dialog opens. Set: Patient Status = "All", Date Range = "All Time". In the Fields section, make sure these are included: First Name, Last Name, Cell/Mobile Phone, Home Phone, Email, Last Exam/Last Visit, Patient Status, Primary Insurance.

• Patient Status: All (active + inactive)
• Date Range: All Time
• Fields: First Name, Last Name, Cell Phone, Home Phone, Email, Last Exam, Patient Status, Insurance
• Click Run or Generate

After the report generates in the preview window, click the "Export" button or go to File → Export. Select CSV format. Save the file and upload to MedMasters via the secure link.
