Log in to ChiroFusion
Navigate to chirofusion.com in your browser and click "Login" in the top-right corner. Sign in with your admin or office manager credentials. ChiroFusion is fully web-based — no software installation. After login you will see the ChiroFusion dashboard with appointment and billing information.
Note:
ChiroFusion does NOT include Last Visit Date in the standard patient export. You MUST also export the Appointment Report to get this data. Both files are required.
Tip:
This export requires running TWO separate reports (patient demographics + appointments) because ChiroFusion does not include last visit date in the standard patient list. Plan for about 12 minutes total.
Export #1: Patient Demographics List
Go to Reports → Patient Reports → Patient List. This report contains names, contact info, and patient status but NOT last visit dates. Configure filters: Status = "All" (active + inactive), Provider = All. Click "Export to CSV".
Export #2: Appointment Report for Last Visit Dates
Now you need the second file. Go to Reports → Appointment Reports. Look for a report called "Last Appointment by Patient" or "Appointment History". This report will provide the most recent appointment date for each patient.
Note:
Last Visit Date cannot be found in the Patient List report — this Appointment Report is the ONLY way to get it from ChiroFusion.
Configure and Export the Appointment Report
In the "Last Appointment by Patient" report: Set Date Range = "All Time" (to capture each patient's most recent appointment). Make sure Patient ID is included in the output (this is the key field for merging with the demographics file). Click "Generate" then "Export to CSV".
Tip:
Including Patient ID in both files allows MedMasters to merge them accurately even if a patient's name changed.
Send Both Files to MedMasters
Upload BOTH files (patient_demographics.csv and appointment_history.csv) to MedMasters using the secure upload link. In your message to your account manager, state: "This is a ChiroFusion export. File 1 = patient demographics (names, phones, emails, status). File 2 = appointment history (last visit dates and visit counts). Both files include Patient ID for merging."
Note:
Both files are required — do not send only one. MedMasters needs both to build the complete patient record.
Tip:
Patient ID is the key field that links the two files. Confirm it appears in both exports before sending.
